Appointments and Forms

In order to schedule your first appointment, please click here. Our office will verify your insurance benefits and will contact you to schedule an appointment. Once the session has been scheduled, to get the most of your sessions, please arrive on time. Sessions are 45-50 minutes, so if you arrive late, you will have less time in which to work.

For private pay clients, Dr. Chauhan is also offering sessions by telephone, Facetime, or Skype. For telephone appointments, you will call Dr. Chauhan’s number at the scheduled time at 972 840-9900. For Facetime sessions, you will call Dr. Chauhan at 857 544-5068, and at pushpachauhan on Skype.

Please note these are only available for private pay clients, as insurance will not cover these types of sessions.

Appointments are charged at the time of booking and are nonrefundable unless a 48 business hours notice is given by phone to reschedule or cancel the appointment.

Forms

For your convenience and to expedite the intake process, you will find below a number of forms which you may print off and complete in advance of your visit to our office. If you are unable to print the forms in advance, we will have these forms available for you at the office. In this case, please arrive half an hour earlier to your scheduled appointment time, so that you will have ample time to complete the forms.

Please read the documents carefully, fill out the forms legibly, add your signature where required, and bring all the necessary forms with you to your first appointment. You will also need your driver’s license, your insurance card, and payment.

For adult patients, please fill out forms 1, 2, and the first page of 5.

For parents scheduling children, please fill out forms 1, 3, 4 and the first page of 5.
